Who we are

CuraSmile PH Advanced Dental Group operates this website and the clinic at 5/F C2 Building, Bonifacio High Street Central, 7th Avenue corner 28th Street, Taguig City, Metro Manila. We are the personal information controller for the data described below, as that term is used in the Data Privacy Act of 2012 (Republic Act No. 10173).

What we collect

When you book an appointment through this website, we collect the details you enter in the form:

• Your full name
• Your email address
• Your mobile number
• The service you are booking, your preferred dentist, and your chosen date and time
• Any notes you choose to add

We also collect standard technical information that your browser sends to any website, such as your approximate location, device and browser type, and the pages you visit.

Please do not enter detailed medical or health information in the notes field. Bring anything sensitive to your appointment instead, where it is recorded in your clinical file.

How we use it

• To schedule and confirm your appointment, and to contact you about it
• To answer enquiries you send us
• To keep the website working and to understand how it is used
• To measure the results of our advertising, as described below

We do not sell your personal information, and we do not use it for marketing messages you have not asked for.

Cookies and advertising measurement

This website uses the Google tag to measure the performance of our advertising on Google. It sets cookies that tell us when a visit to our site followed one of our ads, and when that visit led to a booking or a phone call.

We use a Google feature called enhanced conversions. When you complete a booking, the email address and mobile number you entered are converted into an irreversible code (a SHA-256 hash) in your own browser before anything is sent. Google uses that code only to check whether the booking can be matched to a Google account, so that our advertising reporting is accurate. Your email address and mobile number are never sent to Google in readable form, and Google does not receive the reason for your visit or any notes you wrote.

You can opt out of personalised Google advertising at myadcenter.google.com, and you can block or delete cookies in your browser settings. Blocking them does not affect your ability to book an appointment.

Who else handles your data

We use trusted service providers to run this website and our booking system. Booking details are stored using Google Firebase, and appointments are added to our clinic calendar in Google Calendar. These providers process data on our instructions, and their servers may be located outside the Philippines. Advertising measurement data is handled by Google as described above.

We disclose personal information to anyone else only with your consent, or where the law requires it.

How long we keep it

Booking records are kept for as long as needed to provide your care and to meet our record-keeping obligations, and are then deleted or anonymised. Advertising measurement data is retained according to Google's standard retention periods.

Your rights

Under the Data Privacy Act you have the right to be informed, and to access, correct, object to the processing of, erase or block your personal information, as well as the right to data portability and the right to complain. To exercise any of these, contact us using the details below. You may also raise a concern with the National Privacy Commission at privacy.gov.ph.
